2014 IACUC Protocol Application Form
Use this form to initiation the application process for a new animal research protocol and new submission after your project has completed the three year renewal cycle. Also send with your application a copy of the entire research personnel and Principal Investigator’s IACUC CitiProgram Certification and their Animal Worker Statement of Fitness Form.

IACUC Annual Renewal Form
Use this form to initiate the protocol renewal process prior to your study expiration. Please submit a copy of your IACUC CitiProgram Certification if you had to renew the certification.

IACUC Amendment Request Form
Please submit this form if you have any changes to existing approved protocols. Any changes require the approval from the Institutional Animal Care and Use Committee prior to implementation.

Personnel Change Form
Please submit this form if you wish to amend personnel currently named on a protocol. Changes in the Principal Investigator or co-Principal Investigator are reviewed by the full committee, either at a convened meeting or by designated review.

Animal Work Statement of Fitness
Please complete this form and submit along with IACUC CitiProgram Certificate. This form should be completed during the initial application and if there is an additional personnel being add via the Personnel Change Form. This form should be signed by the person completing the animal worker form and a medical professional. The form should have the medical professional stamp on the document as well.
